Fistula in Ano is an abnormal communication between the anal canal and the perianal skin, usually lined by granulation tissue. In some cases, secondary tracts may also be present.

The most common cause of fistula is explained by the Cryptoglandular Hypothesis. According to this, most fistulas originate from infections in the anal glands. These glands are located between the internal and external anal sphincters and drain into the anal canal.

The infection may begin in the anal glands and gradually spread into the muscular wall of the anal sphincters, leading to the formation of an anorectal abscess. Over time, this abscess may develop into a fistula in ano.

Fistula in Ano can also develop due to other causes such as trauma, malignancy, radiation, tuberculosis, and inflammatory bowel diseases.
